To make an appointment, call (720) 777-1234 and ask for the Center for Gait and Movement Analysis.

Referrals for Gait Analysis

All appointments to CGMA do require a referral from your doctor. The referring doctor must complete the following form and submit it to CGMA in order to complete the scheduling of your appointment and help ensure payment by your insurance. Gait Analysis Referral Form (.pdf)

Patient Questionnaire

Before your appointment, you should complete the appropriate questionnaire and bring it with you to your appointment. This information is necessary before we are able to begin your gait analysis. If you have questions about which questionnaire to complete, please ask when you schedule your appointment.
