It is the policy of Children’s Hospital Colorado that the planning committee and faculty disclose real or apparent conflicts of interest relating to the topics of the educational activity and also disclose discussions of unlabeled/unapproved uses of drugs or devices.   Detailed disclosure will be provided to the learners.

Disclosure Form for Professional Education   – Disclosure of Relevant Financial relationships is required by all committee who develop content and faculty who present at CME designated activities.   Disclosure must be signed prior to the activity.  Faculty who refuse to disclose prior to are eligible for disqualification from the activity.

Conflict of Interest (COI) Resolution Form   – Disclosure of Relevant Financial relationships requires appropriate intervention to remedy a potential conflict. COI Resolution form is required for faculty who have relevant disclosures.
