We value your expertise and appreciate your time and dedication to professional continuing education, and lifelong learning at Children's Hospital Colorado. The Accreditation Council for Continuing Medical Education (ACCME) requires CME providers to identify and resolve conflicts of interests for all individuals responsible for the development, management, presentation, and/or evaluation of a CME/CE activity. In order to fulfill this requirement, disclosure is required.
Faculty refusing to disclose are eligible for disqualification from participation. Detailed disclosure will be provided to the learners.
Purpose: To assure fair and unbiased presentation of professional educational materials and to comply with the ACCME Standards of Commercial Support.
Who: Planning committee members, speakers, presenters, moderators, panelists, authors and/or anyone in a position to control content of a CME activity.
How Often: Annually or within 30 days of terminating or acquiring a new relevant financial relationship.

Until then, please complete and submit the Financial Disclosure Form prior to the activity date. Your participation is dependent on receipt of disclosure and resolution of conflicts of interest.
Faculty with reported disclosures must read, sign and return the Conflicts of Interest (COI) Form for further resolution. Reported disclosures does not disqualify faculty, however, If COI absolutely cannot be resolved, individual will be disqualified from participating.
