How to refer a patient to a Children's specialist

Referring community providers and Children's specialists are partners in the care of patients. We encourage families to coordinate with their primary care pediatrician or family physician before coming to Children's Hospital Colorado. Children's ambulatory (outpatient) services encompass more than 32 different pediatric specialties, with locations at the main campus in Aurora and our Network of Care locations across Colorado.

Phoning in a referral

A community provider can refer a patient to a specialist at Children's by phone by calling One Call at (720) 777-3999, or toll free at (800) 525-4871. Your call will be answered by a nurse who will collect brief demographic and clinical information. You may be asked to fax additional clinical data to optimally prepare for the appointment. For some conditions, our specialists may require more detailed triage information to ensure your patient is referred to the most appropriate provider.

Faxing a referral

Instead of phoning in a referral, community providers and their staff can also fax referral information to Children's in advance of a specialty appointment. For referral fax numbers, please refer to our Directory of Services.

Family appointment requests

Community providers can also instruct a patient's family to contact Children's to schedule the appointment at (720) 777-1234. Patients can also use Children's Hospital Colorado's online appointment request form.

How to directly admit a patient to Children's

A referring community provider can directly admit a patient to Children's Hospitalist Program by calling One Call at (720) 777-3999, or toll-free at (800) 525-4871. Please request the patient placement department.

The referring community provider will be notified with a fax and a phone call at the time of admission of their patient by the emergency department and the inpatient unit. The referring community provider will then be called with any major changes in care during their patient's stay, and will be notified via fax and phone call at the time their patient is discharged from Children's Hospital Colorado.

How to transfer a patient to Children's

A referring community provider can request to transport a patient to Children's Hospital Colorado by calling One Call at (720) 777-3999, or toll-free at (800) 525-4871.

Children's Hospital Colorado has partnered with Flight For Life for pediatric critical care transports from hospitals throughout the region. Our physicians assist with mobilizing Flight For Life during the patient acceptance process. We have also partnered with Pridemark Paramedic Services for our Network of Care interfacility transports, and recommend them for transport from physician offices in Colorado's Front Range communities.
