The more information you can provide about your child’s eye condition and treatment, the more comprehensive second opinion we can provide. Before your second opinion call, make sure to have your child’s information and medical records available:
Diagnosis: Your child’s diagnosis or suspected diagnosis and treatment plan
Diagnosing doctor: The diagnosing doctor’s name, address, phone, and fax numbers
Operations: Any surgeries your child has had
Health insurance status and information
Where will my child be seen for their in-person appointment?
