Diagnosis: Possible SLE (lupus)

What should the referring provider send?

  1. A complete list of all medications
  2. Medical records regarding relevant past evaluation and treatment including outside consults
  3. Relevant laboratory studies (usually should include at least CBC, ESR, ANA panel, metabolic panel, C3 and urinalysis)
  4. Results of any additional tests

Diagnosis: Possible Raynaud’s Phenomenon

  2. Medical records regarding relevant past evaluation and treatment including outside consults
  3. Relevant laboratory studies (could consider obtaining CBC, ESR, ANA panel, PTT, RVVT, anticardiolipin antibody, anti beta 2 glycoprotein I antibody and urinalysis prior to visit if positive family history, severe symptoms or systemic manifestations)
  4. Results of any additional tests
