The more information you can provide, the better treatment plan we can make. Before your second opinion call, please have as much of the following information or records available as possible:

Diagnosis: Your diagnosis or suspected diagnosis and treatment plan
Diagnosing doctor: The diagnosing doctor’s name, address, phone and fax numbers
Operations: If your child has already had surgery, what procedure they’ve had

Your location
Health insurance status and information

Where will my child be seen for their in-person appointment?
