Good Faith Estimate Disclosure

For uninsured/self-pay patients:

Patients who don’t have insurance or who are not using their insurance have the right to receive a “Good Faith Estimate” explaining how much medical care will cost. This applies to appointments that are scheduled three or more days in advance. Upon scheduling your procedure or service, Children’s Colorado will send you a Good Faith Estimate in writing. If you have any questions about your Good Faith Estimate or the amounts billed, please contact the patient cost estimate specialists at 720-777-0720 Monday through Friday, from 8 a.m. to 4:30 p.m. or through email anytime.

In addition to getting an estimate through our online tool, patient cost estimate specialists at Children's Colorado can also calculate the likely amount that is your family’s responsibility based on the scheduled service(s), insurance coverage and specific benefits, such as deductible and co-insurance. Please contact us if you have an appointment already scheduled, and know that these are just estimates and not guarantees of cost.

Estimates provided are only related to Children's Colorado charges, and do not include any bills you may receive from your physician or other providers who provide services to you while at a Children's Colorado location.

First, we strongly encourage you to consult with your health insurer to determine accurate information about your coverage and benefits, and to ensure that the services scheduled are "covered services." You may also need to contact your child's primary care provider's office to obtain specific diagnosis or procedure description.

Once you have that information, here are examples of the types of services we can provide estimates for:

  • Imaging, such as MRI scans, CT scans, X-rays and ultrasounds
  • Echocardiograms
  • Holter monitors
  • Office visits with specialists
  • Lab tests
  • Physical and occupational therapy
  • Partial hospitalization program
